Terms & Conditions

Please read these Terms and Conditions ("Agreement") carefully before using the mobile phlebotomy services provided by Vein Express Mobile Phlebotomy ("Service Provider"). This Agreement sets forth the terms and conditions for obtaining mobile phlebotomy services.

1. SERVICES: Service Provider offers mobile phlebotomy services, including blood draws and specimen collection, conducted at the location of the client's choice.

2. APPOINTMENTS: a. Clients must schedule appointments in advance through the booking site at www.veinexpress.com. b. Clients must be available at the specified location at the scheduled time.

3. CLIENT RESPONSIBILITIES: a. Clients must provide accurate information about their health and medical history. b. Clients must follow any pre-collection instructions provided by the Service Provider.

4. PAYMENT: a. Payment for services is due at the time of the appointment. b. Accepted payment methods include Cash Pay and all major Credit Card and Debit Card. c. Prices are subject to change, and clients will be informed in advance of any changes.

5. CANCELLATIONS AND RESCHEDULING: a. Clients must provide at least 24 hours’ notice for cancellations or rescheduling. b. Late cancellations or no-shows may be subject to a 25.00 fee.

6. CONFIDENTIALITY: a. Service Provider will maintain the confidentiality of client health information in accordance with applicable laws, including HIPAA. b. Client information will only be disclosed to third parties as required by law or with the client's consent.

7. COMPLIANCE WITH LAWS: a. Service Provider will comply with all relevant laws and regulations governing the provision of phlebotomy services. b. Clients are responsible for complying with any applicable laws or regulations related to their health information.

8. LIMITATION OF LIABILITY: a. Service Provider is not liable for any injuries or complications arising from the phlebotomy procedure, provided that it is performed in accordance with accepted standards of care. b. The Service Provider is not liable for delays or cancellations due to circumstances beyond its control, such as traffic or inclement weather.

9. INDEMNIFICATION: Clients agree to indemnify and hold the Service Provider harmless from any claims, damages, or liabilities arising out of the client's use of the mobile phlebotomy services.

10. TERMINATION: Service Provider reserves the right to terminate services if clients violate any terms of this Agreement.

11. GOVERNING LAW: This Agreement is governed by and construed in accordance with the laws of Louisiana.

12. AMENDMENTS: Service Provider reserves the right to amend this Agreement at any time. Clients will be notified of any changes.

By using the mobile phlebotomy services provided by Vein Express Mobile Phlebotomy, clients acknowledge that they have read, understood, and agree to be bound by these Terms and Conditions.
